“This is an utterly heartbreaking case and my thoughts are with Maeve’s family and friends, particularly her daughter, whose voice comes through so powerfully in this report.

“Victims of crime do not fit neatly into departmental siloes and this tragic case clearly demonstrates the importance of our health service in supporting victims, and indeed whole families, in a holistic and joined up manner. 

“Maeve’s daughter’s grief is palpable and these DHRs make for difficult reading. However, it is important that we do read them, we acknowledge any shortcomings in our prevention efforts, and we take swift action to implement any learning so we can prevent such tragedy happening again.”
